Connect Notion to Auto-Log Responses

Open a form's Integrations tab and click Connect Notion. You'll sign in to Notion and, on its consent screen, choose which page to share with Orbiform — Orbiform only ever sees that page (and whatever it creates under it), never your whole workspace. Orbiform then creates a new database under that page, named after your form, with one property per question.

Every time someone submits the form, Orbiform adds one page to the database with the submission time and each answer, matched to the properties created at connect time. If you add or remove questions afterward, existing properties aren't reshuffled — disconnect and reconnect to regenerate the database with your current questions.

Click Disconnect next to the connected database in the Integrations tab. This only stops new pages from being added — the database itself, and everything already in it, stays exactly as it is in your Notion workspace.

Each form still gets its own database, but you only need to sign in to Notion once per workspace — connecting a second (or third) form doesn't require re-authorizing. From that form's Integrations tab, pick your already-connected Notion account from the dropdown instead of starting a new sign-in, or add the connection from Settings → Integrations and choose which form to attach it to — the same account is reusable across as many forms as you like. To collect multiple forms into a single database, export each form's responses to CSV and combine them manually, or use a webhook to a tool like Make or Zapier that can route multiple forms into one database with custom logic.

See every Notion account connected to your workspace, and how many forms each one is used by, under Settings → Integrations → Connected apps. Each account is listed once, with every form using it nested underneath it — so it's clear at a glance which databases share the same account. Removing an account there disconnects it from every form using it, not just one — the confirmation dialog shows exactly how many forms will stop syncing before you confirm.
